REQUEST FOR QUOTATION/PROPOSAL

RFQ/P 008-2017/18

Quotations are hereby requested for the Supply, branding and delivery of the CHDA Corporate wear according to the following specifications by 12h00 on 27 September 2017:

The closing date and time for the submission of quotations is Wednesday, 20 September 2017 at 09h00

OTHER REQUIREMENTS:

Business profiles which indicate the track record / experience of the service provider with similar projects & with contactable references must be submitted with the quotes proposals;
Catalougues/ clear coloured pictures (of available/accessible stock) / samples which indicate the required goods must be submitted with the proposals;
Clear fabric types which are offered must be indicated on the proposals;
The proposals must indicate commitment/ ability / capacity to deliver the goods by the 27^th of September 2017 if the order is awarded by 20 September 2017.

Suppliers must take particular note of the following:

A pre-evaluation based on the above paragraph requirements qualification will be conducted before any proposal and quotation is considered for the 80/20 preferential points system evaluation
Selection for award will be based on capacity to deliver as per the specifications and quality offered versus cost effectiveness (therefore for this particular RFQ CHDA reserves the right for not awarding only based on the basis of the highest preference points scored)
CHDA RESERVES THE RIGHT to award either for one of the above options or for a combination of the above options
Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report (If it was never submitted before)
Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company (If the MBD 4 Form was never submitted with the Supplier Database forms)
Submit the valid B-BBEE certificate (If it was never submitted before)
Submit a valid original Tax Clearance Certificate (If it was never submitted before)
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
