  • Training to be conducted in person for 2 days (dates to be communicated) at CHDA offices.
  • The service provider should provide training material.

Suppliers must take particular note of the following:

  • COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE.
  • Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report.
  • Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company.
  • Submit a valid Tax Clearance Certificate /Pin
  • If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
  • Directors ID(s) to be provided.
  • valid proof of address/lease agreements/rates account statement etc to be provided.
  • Certification document from a medical practitioner as proof of disability.
  • Service provider to provide 2 proofs of rendering a similar project like PO, appointment letters etc for reference purposes and, we expect the service provider to issue certificates after the training.

Evaluation criteria (Only applicable to quotations equal to/ above R2 000.00)

  • This is an 80/20 preference points system.
  • Price is allocated up to 80 points & 20 points will be allocated as follows:
  • Black ownership (Max. 10 points)
  • 10 points for black owned company, confirmable by proof of CSD registration and certified ID copy(ies) of entity’s director(s)
  • Nil points for no black ownership
  • Locality (MAX. 5 points)
  • 5 points will be allocated to service providers within CHD jurisdiction, confirmable by valid proof of address/lease agreements/rates account statement
  • Nil points outside CHD jurisdiction.
  • Youth ownership (Max. 2.5 points)
  • 5 points, confirmable by proof of CSD registration and certified ID copy(ies) of entity’s director(s)
  • No points if above age of youth
  • Disability (Max. 2.5 points)
  • 5 points for the people living with disabilities, confirmable by certification document from a medical practitioner.
  • Nil points for people with no disabilities.
