REQUEST FOR QUOTATION

Quotations are hereby requested for SKILLS PROGRAMME TRAINING FACILITATION - Advanced certificate in Business Management and Administration -NQF level 4. (According to the following terms of reference/specification):

Description of conditions:

The training provider will facilitate training of the skills programme as per the service level agreement.
The minimum number of credits per program is 20
Certificate of compliance will be required on completion
CHDA will be responsible for catering and organising a training venue in Queenstown.
The duration of training may not exceed 3 weeks.

Requirements:

Service provider to have a NQF level 4 SAQA Accreditation in the business management/administration programme.
Service provider to bring own training material for distribution and teaching.

Proposals/quotations must be emailed to kwanele@chda.org.za

Suppliers must take particular note of the following:

COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE;
Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report
Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company
Submit the valid B-BBEE certificate
Submit a valid Tax Clearance Certificate /Pin
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.

Evaluation criteria (Only applicable to quotations equal to/ above R 30 000.00)
