REQUEST FOR QUOTATION

Quotations are hereby requested for SIM cards, connection and 10 GB data per month for CHDA Board Members for a period of 24 months according to the following specifications / T.O.R.:

Once-off SIM card supply and registration
10 GB data per month, with top-up option available for 24 months
Total of 15 SIM cards only
Services must be rendered within 7 days from the date of the official Purchase Order.

Suppliers must take particular note of the following:

COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE;

Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report
Service providers should also complete & submit the MBD 4 form to declare the interest of the Directors of the Company
Submit the valid copy of a B-BBEE certificate / B-BBEE Sworn Affidavit
Submit a valid copy of a Tax Compliance Pin document
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
