REQUEST FOR QUOTATION

Quotations are hereby requested for Supply and Delivery of white Domestic Weaners according to the following terms of reference/specification:

Supply and delivery of 100 large white domestic piglet “Already vaccinated, with medication pack. The pack must have:

2 External & Internal parasite remedy (3x Ecomectin 1% Injectable)

3 Wound treatment (3x Futaspray 400ml )

Suppliers must take particular note of the following:

STANDARD COMPULSORY SUBMISSION DOCUMENTS, EVEN IF THEY WERE SUBMITTED BEFORE;
Written Commitment of rendering the required services within 07 days after official appointment must be provided
The required stock to be delivered to Bilatye & Mkhonjane Secondary co-op +-5km out of Qamata on the route towards Queenstown direction coming from Cofimvaba and Coming from Queenstown toward Cofimvaba its +_ 63.6km
Submit a copy of the National Treasury Central Supplier Database (CSD) registration summary report
Service providers must complete & submit the MBD 4 form to declare the interest of the Directors of the Company
Submit the valid copy of the B-BBEE certificate or a Sworn Affidavit for B-BBEE Level of status of contribution
Submit a valid copy of a Tax Clearance Certificate / of a SARS Tax Pin document
If a supplier is not a VAT vendor, VAT must not be included, otherwise all the prices must include VAT.
